Rental Instructions

Sign and return the Confirmation Form with a check for $100.00 to the MDA office to reserve the Pavilion for your selected date.

Sign and return the Pavilion Lease Agreement and Condition of the Pavilion forms at least four weeks prior to your event.

If serving alcohol, complete the Alcohol Permit Waiver form and submit payment in the amount of $10.00. In the event alcohol will be sold at the event, ensure the caterer has an endorsement for the specified event or obtain an alcohol permit from the Department of Revenue Liquor Division.

Pay the balance of the rental fee and submit a $250.00 Security Deposit check four weeks before your event.

Arrange a time to meet with MDA facilities staff to discuss your layout preferences prior to your event.
